The hallucination was born out of House's guilt over Kutner's death; which in turn caused his chronic insomnia-induced insanity and Vicodin addiction. As a subconscious of House, House was terrified of it as the hallucination had been manifesting intents to harm others as well as meticulous homicidal tendencies. However, although it appeared to him as Amber, it actually is a reflection of House himself, allowing him access to some of his subconscious thoughts.
